New Patient
New patients must provide their personal and health-related information for review before we can schedule an appointment. Returning the completed forms by mail or fax allows one of our nutritional consultants time to review them before the appointment. Personal patient information will remain confidential.
Payment is required prior to scheduling an appointment with the nutritional consultant. The new patient forms inlcude pricing for 3 different consultation time frames and a request for credit card information.
